Nuclear medicine is the modality which utilises “open” radioisotopes for evaluating function of organs and also diagnosing tumors ; as it assess functionality of tissue at cellular level. It is used as a source of ‘internal’ brachytherapy thus also having therapeutic applications . Various radioisotopes are available but the most commonly used are gamma emitter which are imaged using a gamma scanners and positron emmiters which can be located using the PET technology. in recent years a hybrid technology is available with the PET scanner called the PET/CT scanner which provides both anatomical and functional information simultaneously. This fusion technology has shown a great potential in the management of malignancies and is incorporated as a standard of care in multiple protocols of cancer care.

The department was inaugurated  in 2004 as the Bio Imaging department. The country’s first hybrid PET/CT scanner was installed here with a foresight of the increasing application of this modality in management of cancer.

This department was unique as it had the services of both the nuclear medicine and radiology specialist working under one roof which helped in building various protocols using the advantages of both the modalities - radiology and nuclear medicine to suit the requirements of the clinicians. Modifications in imaging methods followed by the developed world were done to suit the Indian patient population. It continues to offer this fusion service towards patient care.

We strictly abide by the norms of the regulatory body (Atomic energy regulatory board -AERB). The licence to procure a radioisotope was the limiting factor to increase the patients imaged in the department. To overcome this a novel method of handling large dose of radioisotope by following the ‘lot system of delivery of radio tracer is implemented. This technique enables fractionated doses of the total dose to be delivered at regular intervals which allows permissible radiation limits to be maintained.

The radiation personnel follow strict protocols as per the SOP’s to reduce the radiation exposure. Everyone is provided with TLD’s and dosimeters if necessary to measure the radiation exposure.

All necessary documentation is maintained as per the AERB regulations.

The department underwent an expansion with the installation of a gamma scanner in the department in 2006 to cater to the demand for functional imaging of organs deemed prone to chemotherapy or radiation induced toxicity. The department offers multitude of services to assess functionality of organs and tumors - Tc 99m renal scans for GFR estimation, multi gated acquisitions (MUGA) for cardiac assessment, 99m macro aggregated albumin (MAA) scans for lung function to name few.

Sentinel node detection or biopsies are also performed in conjunction with the surgical departments for breast, gynecological, head & neck and urological malignancies.

A second PET/CT scanner was added in 2011 to provide for the increasing demand of the services. The older scanner was also replaced with a new scanner at the same time. We now perform approximately 1800 scans in a month.

We procure a Gallium generator on a regular basis to use Gallium 68 labelled radio tracer for specific tutor imaging like Ga 68 PSMA (prostate specific membrane protein) for prostate imaging & Ga 68 labelled somatostatin labelled tracer for neuroendocrine tumors.

Many invivo testing service were initiated in the department with the addition of various instruments. 

Providing both in vivo and in vitro service and specialised tumor imaging the department was rightly renamed as the “Department of Nuclear medicine and Molecular imaging” in 2010 as it is know in the present day.

Radionuclide therapies using Phosphorus 32, Strontium 89, Samarium  for bone pain palliation is offered to patients on an OPD basis. 

Low dose 131 -iodine therapies for thyroid cancers are also done on an OPD basis. An isolation ward for radionuclide therapies requiring in - patient services is also being constructed and these services shall be initiated soon.

Yttrium 90 therapeutic beads for Hepatic malignancies both primary and secondaries is provided in conjunction with the interventional radiology department.

The department is connected to the Hospital PACS system fro storage of the data. It also has mini intradepartmental PACS for archival of data for teaching purpose - teaching files. 

Post Graduate education to doctors - M.D. and technologists - D.F.I.T (Diploma in Fusion Technology) affiliated to the HBNI universities are being conducted.

General Nuclear medicine section

1. General Nuclear medicine section

This section of the department is concerned with imaging utilising the gamma scanner. there is a radio pharmacy as per the with a fume hood

Patients undergo following investigations

Renal - camera based GFR, estimation, renal function evaluation - excretory and cortical differential function,

Cardiac evaluation - perfusion scintigraphy using Tc 99m MIBI for myocardial function and multi gated acquisition (MUGA) for LV ejection fraction for cardio toxicity,

Pulmonary - lung function evaluation - perfusion scintigraphy for quantitative estimation; V/Q scans for pulmonary embolism

Skeletal - Bone scans

Hepatobiliary - HIDA scans for hepatic function, biliary leaks, biliary atresia

gastric - gastric emptying studies , gastro oesophageal reflux

vascular - lymphoscintigrapghy , GI bleed

MIBG scans for neuroblastoma

Sentinel node detection - breast, gynecological, head and Neck and urological malignancies.

2. PET/CT scanner sections

These are the places where PET/CT is performed.

18 F FDG PET/CT studies account for 95% of the total scans acquired. Ga 68PSMA and Ga 68 DOTA TOC PET/CT are the there PET/CT scan performed in the department. 18 F FET and MISO are being used for trial purposes.

Both the scanners have a facilities for RT planning PET/CT acquisition.

Scanner 1 region

This section has the 64 slice hybrid PET/CT scanner. the injection room of this area is connected to the cyclotron of the BARC from where the radioisotope 18 F FDG is procured.

Scanner 2 region

This is located in the passage of the basement of the main building. The radio pharmacy of this section has the Germanium - Gallium generator. This section has facilities to better manage patients on trolleys.

In vivo imaging and Medical physics section

3. In vivo imaging and Medical physics section

This section is associated with the in vivo estimation. The section is equipped with well counters, thyroid probe, QA and QC of new labelled radio tracers is done in this part of the departments.

ACTREC

4. ACTREC

This is a department in the ACTREC facility of the hospital located in Khar Ghar, Navi Mumbai. this section is equipped with a 5 ring PET/CT scanner. this department caters to only 18 F FDG PET/CT scans at present. RT planning PET/CT facilities are available here.

Therapeutic section

5. Therapeutic section

This is located adjacent to the gamma scanning section due to need for pre and post therapy evaluation.

OPD procedures for following therapies are provided:
1. low dose 131 - Iodine therapies for low risk thyroid cancer  
2. Bone pain palliation using P32/Sr 89/Sm 

Post treatment scans are obtained for these patients to document deposition of therapeutic tracer.

Y90 beads are injected in the patient in the interventional radiology department situated close by and patients are shifted to the gamma imaging section for post therapy imaging.

Phase 1 trials are under process for evaluation of labelled antibodies  for therapeutic purposes namely 131 - I Rituximab and Lu 177 Herceptin. Radio kinetics, uptake studies and dosimetry is being done in the department related to these trials.

Observership programme in PET/CT –

6 months observership in PET/CT imaging – Comprehensive training programme for post graduate doctors, radiologists and radiation oncologists who want hands-on experience of doing and reporting PET/CT. The course includes training in FDG PET/CT, 18F- Fluoride PET/CT, 68Ga- PSMA and DOTA imaging.
