Information regarding the  procedure and charges for TMH opinion on surgical pathology referral material

To Whomsoever it may concern
If you wish to refer material for a histopathology opinion please send pertinent and relevant details of the patient's name, age, sex, address, telephone number and e-mail address together with the clinical history, duration of illness, & site of biopsy. 

If your tissue/material is related to a bone or CNS lesion, kindly furnish the X-ray films and radiological diagnosis by post or courier service.

All payments should be sent by a single D.D. in favour of “Tata Memorial Hospital, Department of Pathology.” Payable at Mumbai. Please send individual D.D.’s for each patient.

The revised histopathology charges  including immunohistochemistry w.e.f 1/04/2012 are as given below : -

The following details are essential:

• Patient’s name
• Age/Sex
• Full Postal address (reports are routinely despatched by courier)
• Telephone number/ Fax Number/ E-mail address (if available)
• Clinical details
• Site of disease, nature of material and details of previous surgery/ biopsy
• Radiological images with radiology reports.  These are essential for Bone soft tissue and CNS cases
• Results of hematological and biochemical tests, including tumour marker levels if relevant

The physician requesting the tests may please forward his complete address, contact number and e-mail ID to assist the reporting process 
A single demand draft for each patient may be drawn in favour of “Tata Memorial Hospital, Department of Pathology.” There are no extra charges for Immunohistochemistry.
Separate demand drafts are required for each case referred.
Slides/ blocks prepared elsewhere and submitted for opinion are retained by the Department.

On request, reports can be forwarded by Fax or E-mail.
