The Department of Pathology, Tata Memorial Hospital has started EQAS in Histopathology since January 2016.
Details are given below:
We conduct 3 cycles per year in this program which include technical as well as interpretative aspect. This includes only neoplastic cases from different system specialties.
Please go through the following information
Participant will be charged for this program by one time enrollment fee Rs. 5000/- and yearly renewal fee Rs. 5000/- + 18% GST i.e.1800/-
(Total amount is 11800/-) amount which is Non-refundable & Non-transferable.
The Demand Draft of Rs. 11800/- should be made in favor of "Tata Memorial Hospital”, Payable at Mumbai and courier it to the address given below.
Renewal charges have to be paid in December every year for the continuity of EQAS membership for the next year.
Your membership for this year will be considered only after receiving your DD.
Kindly mark your envelopes / email with "EQAS Diagnostic Histopathology" while sending any EQAS correspondence to TMH.
Along with the Demand draft please send the details of authorized signatory like Name of the pathologist, Qualification, Designation, Mailing address, Email ID, Mobile No. etc. in the attached format.
Please note that, only one authorized signatory will be considered from each registered Lab/Hospital/ Institute.
