1. Tata Memorial centre intends to enter into Rate Contract for the supply of Smart Cards in the Hospital, Parel & ACTREC, Kharghar, Navi Mumbai, HBCH (Sangrur at Punjab), HBCH (Mullanpur-Punjab), HBCH (Vizag, Vishakhapattanam) and HBCH & MPMMMCC (Varanasi)
2. A set of documents containing Technical specifications, General instructions and Terms and Conditions can be obtained from 11-12-2020 to 11-01-2021 from the office of the Purchase Officer, 4th floor, Service Block, Tata Memorial Hospital on all working days except Saturdays and Holidays between 10.00 a.m. and 4.30 p.m. on payment of Rs.500/- for each tender by cash (non refundable) at 1st floor, Homi Bhabha Bldg, cash counter Tender document charges for the tender forms downloaded from our website to be paid at the HBB Cash Counter before its submission. Tender fees will not be accepted through M.O. or Bankers Cheque/ DD by post. Tenders addressed to the Director, Tata Memorial Centre, Dr. E. Borges Marg, Parel, Mumbai – 400 012 should be submitted in two parts simultaneously in two separate sealed envelopes duly superscribed as Tender No…… with due date “Part I and Part II”. Part I covering technical bid and Part II covering price bid. Both Part I and Part II tenders should reach on or before 12-01-2021 in Purchase Department upto 2.00 p.m. Technical bid will be opened on 13-01-2021 at 10.30 am onwards in front of those tenderers who wish to be present. Time and date of opening of the Part II (Price Bid) will be intimated only to the tenderers who have qualified after evaluation of Part I (Technical Bid).
3. Samples in 10 Nos. of preprinted smart card and plain cards should be submitted to the Stores Dept, Basement, Main building. Tata Memorial Hospital. The vendor must submit samples with a label specifying the Schedule No. item code, name of the article, Brand name and the name of the vendor.
4. Tender document duly signed and stamped on all pages with original receipt of EMD for 'Purchase of Tender' & original receipt of 'Earnest Money Deposit for Rs. 10,000 (Rupees Ten thousand only) in sealed envelope superscibed with "Tender for Rate Contract 2021-2023 of schedule No…." & EMD receipt number, addressed to the Director, TMC, Dr. Earnest Borges Marg, Parel, Mumbai 400 012 may be delivered to the Purchase Department on all working days except Saturday & Sunday upto 2.00 pm on or before 12-01-2021. In case the date happens to be an unscheduled holiday, the next working day will be taken into account for purchase of tender and its submission.
5. It is responsibility of the bidders to see that the completed bidding documents are deposited in the Tender Box kept inside the Purchase Department, Tata Memorial Hospital, Parel, Mumbai–400 012 on or before the date and time mentioned above for submission of tender,failing which the bid would be considered late and rejected. Mere handing over of the bidding documents at reception counter or at any other counter or room or person cannot be considered as submission of bid.

If the Tender Document is downloaded from website vendor has to pay Rs.500/- as Tender document charges (non refundable) besides the EMD amount Rs.10,000/-(Refundable) is to be paid in the Cash counter, HBB, 1st floor, TMH, Parel before submitting the Tender.
