1.  A committee for empanelment may be constituted which will visit the centres and verify the details of the machine, manpower and infrastructure.
2. The Proposed Rate Contract for Empanelment will be for a period of 2 years and the MOU will be for maximum of 2 years’ subject to TMC rules and regulations.
3. The tender will be in the form of Expression of interest and there will be 3-4 vendors will be selected in the panel who fulfills the technical parameters.
4. The proposed bidders (centres) will not quote procedure charges. The CT MRI Imaging charges for general and private category to be as per the TMH Schedule of charges.
5.  The referrals will be made for those patients whose appointment are more than 7 days waiting period.
6.  The patients will be referred to em-panel centres sequentially (1,2,3….) basis which will be operate on automated system.
7.   However, Radio Diagnosis Dept. will have the full rights to refer any patient as per the situation or case.
8.  Of the total referrals to the centre, at least 50% will be from General category patients.
9.  The consumables required for CT/MRI procedure will be charged by Empanel centre(s) as per TMH rates prevailing.
10. The empanel centre(s) cannot use this agreement and name of TMH for any sort of Branding, Advertisement or any commercial gains.
11.  Since the tender will be in the form of Expression of interest and multiple vendors will be empaneled, EMD need not be taken from the probable bidders.
12.  Performance Bank Guarantee Rs.2.00 Lakhs to be taken from the finalized Empanel vendors after finalization of contract and/or release of work order/allotment letter.
13.  A Pre bid meeting to be held to clarify terms and conditions of the subject tender.
14.  Centre commits to provide scanning service to TMH referred patients
15. The Centre should submit i) Expression of Interest Form (Annexure-I) duly filled in and ii) Technical compliance statement (Annexure-II) duly filled in enclosed herewith. TMH reserves the right to reject the Expression of interest (EOI) if the Annexure-I and Annexure-II duly filled in is not submitted in the enclosed format. 

Interested Imaging centres with necessary documents [Expression of Interest, GCP certification, ISO certification, NABH/NABL accredited Certificate copy should submit their Expression of Interest alongwith i) Expression of Interest Form (Annexure-I) and ii) Technical compliance statement (Annexure-II) in the Purchase Dept, Tata Memorial Hospital, Parel, Mumbai -400012 by personally or by Regd. Post/Speed post / courier so as to reach on or before 07/08/2020 upto 2.00 pm. All applications received will be scrutinized, compiled and will be sent to Competent authority for further needful in a reasonable time. This is an expression of interest for the subject services and not an invitation to bid.

The EOI should be complete in all respects and signed on all pages.

The Expression of Interest (EOI) should be delivered in a sealed envelope and the envelope shall super scribe with “Expression of Interest (EOI) for Empanelment of Scan Centres for CT/MRI procedure referred by TMH and for TMH patients”

The Expression of Interest (EOI) may be submitted by the Owner/Managing Trustee or His/ Her authorized representative of the Imaging Centre. Representative will have to enclose the letter of authority along with expression of Interest.
