The following documents are required for Physical/ Online Reporting (for uploading) for verification by the College authorities:

 

i.  Copy of downloaded NEET PG Admit Card.

ii.  Any Photo Id proof (Aadhar Card/Driving License/ Pancard/Passport)

iii.  NEET PG-2020 marksheet

iv.  Result/Rank Letter issued by NBE

v.   Nationality Certificate/valid passport or School Leaving Certificate of HSC/12th including the nationality of the candidate as “Indian”.

vi.  MBBS Degree/ Passing Certificate

vii. Internship Completion Certificate from university / the head of the institution or Certificate indicating likely date of completion of internship from the Head of          the institution (as per Annexure J in NEET PG Brochure)

viii. Permanent / provisional Registration Certificate of Maharashtra Medical Council or other state medical councils in India/MCI.

ix.  Certificate from Head of Institute showing that the Medical College/Institute from which the candidate has passed MBBS examination is recognized by Medical        Council of India.

x.  Medical Fitness Certificate

xi.  Mark Sheets of MBBS- 1st, 2nd & 3rd Professional Examinations.(asked in All India Quota)

xii.  High School/Higher Secondary Certificate/Birth Certificate as proof of date or birth.

xiii.  The Candidate should also bring/ upload the following certificates, if applicable

       a) Caste certificate

       b) Caste validity certificate

      c) Non Creamy Layer Certificate (as mentioned in Govt. Resolutions and valid upto 31/03/2020)

      d) EWS eligibility certificate

      e) All India Quota / AIIMS / Central Govt. Institutes selection letter/letter from Dean / Principal stating that the candidate was admitted for MBBS under                    15%  quota /AIIMS/ Central Govt. institution from All India Entrance Examination.

      f) Medical Disability certificate for persons with disability (PWD) candidates

     g) Bond Release certificate from DMER, Mumbai

     h) NOC from Directorate of Health Services (for in-service candidates)

     i) Orphan certificate from appropriate authority

 xiv)  Hand Written and Self Attested Undertaking by the candidate stating as under: 

“I hereby declare that all the information given/ uploaded by me in the application is factually correct and true to the best of my knowledge and belief. I undertake that in the event of any information being found false or incorrect at any stage, my candidature is liable to be cancelled and I will have no claim on the seat allotted to me by the competent authority.”

                 xv.)  In addition to the above we require the following documents at the time of physical joining ( For Final Retained students) : 

1. Attempt certificate

2. College Leaving / Bonafide/ Transfer

3. Migration certificate issued by University

4. Self educational gap (If Gap is more than 6 months after completion of internship) affidavit by student on Rs. 100/- stamp paper with notary (if applicable)

5. No Objection Certificate (NOC) / Relieving order from the current Employer

6. Service Bond on stamp paper of Rs. 200/- with notary (mandatory for those who are retaining for Final Admission at Tata Memorial Hospital)

7. Affidavit for change in name – A copy of Govt. Gazette, Marriage Certificate (For Female)

 

Please note online payment of only Rs. 21,000/- will be accepted.

NOTE:

 

  • All admissions made online will be deemed Provisional, subject to verification of documents at the time of Physical Joining and Medical Fitness at the college.
  • Candidate should send email alongwith the allotment letter and receipt/screenshot of payment of fees on account of securing admission.
  • Candidates who wish to retain the seat should email the Status Retention form (as per the format mentioned in NEET PG Brochure).
  • Candidates who wish to cancel their seat; he/she should mail cancellation form to College (from registered email id).
