Health Schemes at HBCHRC Mullanpur Punjab


1. Pradhan Mantri Jan Arogya Yojana (PM-JAY)

  Ayushman Bharat, a flagship scheme of Government of India, was launched to meet Sustainable Development Goals (SDGs) and its underlining commitment, which is to "leave no one behind."Ayushman Bharat PM-JAY is the largest health assurance scheme in the world which aims at providing a health cover of Rs. 5 lakhs per family per year for secondary and tertiary care hospitalization across public and private empaneled hospitals in India.PM-JAY provides cashless access to health care services for the beneficiary at the point of service. It covers up to 3 days of pre-hospitalization and 15 days post-hospitalization expenses such as diagnostics and medicines. There is no restriction on the family size, age or gender.Services include approximately 1,393 procedures covering all the costs related to treatment, including but not limited to drugs, supplies, diagnostic services, physician's fees, room charges, surgeon charges, OT and ICU charges etc.

2. Mukh Mantri Punjab Cancer Rahat Kosh (MMPCRK)

  Mukh Mantri Punjab Cancer Raahat Kosh Scheme was initiated by the Government of Punjab with a view to provide financial assistance to Punjab resident cancer patients. Financial assistance of up to Rs.1.50 Lakhs (One lakh fifty thousand) is provided for the treatment to each cancer patient. First of all, Download Application Proforma and Estimate Proforma for Patients or obtain the same from concerned Civil Surgeon Office then Concerned Hospital should fill the estimated amount for further treatment of patient. Attach a recent photograph on the application form. Residence Proof, Attested copy of Lab Report Confirming a diagnosis of Cancer, Attested copy of details of any Previous Treatment, Self-declaration must be attached with the application form. The Application should be submitted to the District Civil Surgeon Office by the Patient/Relative(s)

3. Ex-Servicemen Contributory Health Scheme

  ECHS has a well-defined procedure for Hospital and clinic empanelment. The Scheme aims to provide allopathic and AYUSH Medicare to Ex-servicemen pensioner and their dependents. The scheme is eligible for any person who has served in any rank (whether as Combatant or as Non-Combatant) in the regular Army, Navy and Air Force of the Indian Union, Gallantry award winners, Members of the military Nursing Service (MNS).An important aspect of ECHS is that, it not only covers the ESM pensioner, but also family pensioners & widows who are in receipt of pension – thereby ensuring their medical care, even in absence of the pensioner.

4. Employee State Insurance Corporation Scheme

  Employees State Insurance Scheme is operated by Employees State Insurance Corporation which is a health insurance and social security scheme. Every card holder can avail cashless treatment under this scheme. They need referral letter from the ESIC for treatment in hospital. All the employers who are covered under the ESI Act have to mandatory register themselves under the ESI scheme. The factories or establishments having more than 10 employees (in some states it is 20 employees) with a maximum salary of Rs. 21,000/- per month are covered under the ESI Act.
