Clinical History: This is a Pap smear of 43-year old peri-menopausal women, presented with complaints of intermittent pain in abdomen and right lower back of one year duration. She had history of exploratory laparotomy and appendicectomy for intestinal obstruction. On clinical examination, mild tenderness was felt over right iliac fossa but and no mass lesion. On examination with speculum, small papillary projection about 3mm was present in ectocervix (not suspicious and vagina appeared normal. Per vaginal and rectal examination cervix was soft, fornices were free and presence of right adnexal hard mobile mass of 6cm wide was identified. Parametrium, rectal mucosa and pouch of Douglas were free. On VIA/VILI, late appearing distinct uptake over a large area occupying all four quadrants were note. Computerized Tomography (CT) scan revealed showed Right adnexal mass and soft tissue density in right ovary, moderate fluid in pouch of Douglas and normal size uterus with endometrial thickness of 12mm.
