Pleural fluid smears of a 21-year old woman presented with complaints of fever and cough since 8-10 days. Computed tomography (CT) scan of thorax, abdomen and pelvis showed large heterogeneously enhancing lesion with multiple cystic areas in the anterior mediastinum with mild loculated pericardial and moderate pleural effusion. Multiple enlarged paratracheal, hilar, lower para-oesophageal and pretracheal nodes were present. The total WBC count was 22,000/mm 3 .
